DESCRIPTION OF SERVICES PROVIDED

 

Criminal and Clinical Polygraph Examinations are conducted at the requests of attorneys, law enforcement agencies, prosecutors, therapists, probation and parole departments. The administered polygraph examinations are used to establish commission of criminal or sexual offenses, re-offenses, obtain examinee full disclosure, baseline behavior, and adherence to regulations, compliance with policy requirements and procedure, or any violation of parole/probation guidelines or treatment rules.

 

PREPARATION AND RELEASE OF REPORTS: All polygraph examination reports are completed containing all information obtained from the examinee during an in-depth and detailed pre-test interview conducted prior to each polygraph examination. The reports are provided to the requester within twenty-four hours. If the polygraph examination is conducted as part of a court ordered treatment program the probation officer and service provider are provided a copy of each report to ensure that all pertinent agencies and individuals are informed of the examination results and behavior of the examinee.

 

VERBAL CONFIRMATION: When deemed appropriate, verbal confirmations of polygraph results are provided to the requester immediately after a polygraph examination.

 

FAX/E-MAIL COPIES: Unless otherwise requested FAX/E-MAIL copies of polygraph reports are provided to all pertinent personnel to ensure timely reporting and notification of the polygraph examination results.

 

WRITTEN REPORTS: A written report of each polygraph examination will be mailed or hand carried to pertinent recipients when a serious reportable violation or condition is revealed during any polygraph examination, pre-test or post-test interview. Written reports will not be released to the examinee based on a simple written or verbal request. If an examinee requests a copy of a polygraph report they will be denied access.

 

ADMISSIONS: All admissions and confessions made during the polygraph testing procedure are documented in the report of the examinee’s polygraph examination.

 

ADMISSIONS STATEMENTS: If an examinee makes a verbal admission, a written statement documenting their admission is obtained or requested to be executed by them prior to their departure from the testing room. The original written statement is forwarded to the requester. If the polygraph examination is administered as part of a court ordered supervision and monitoring program. A copy of each written statement is provided to the supervising officer and treatment provider.

         

CLINICAL POLYGRAPH EXAMINATIONS: Several types of clinical polygraph examinations are performed and each examination has a unique written report that is tailored to the specific polygraph examination. Polygraph examination services are provided and conducted for pre-employment purposes, criminal issues, DUI cases and probation, sex crime violations and monitoring purposes; and, attorney requirements.

 

INSTANT OFFENSE: Covers the examinee’s original offense only if the examinee is in full or partial denial of the facts of the case. A copy of the offense report, victim statement, witness statement and/or summary of the offense should be provided to the polygraph examiner to ensure that appropriate questions are formulated for the requested polygraph examination.

 

MONITORING: Covers the examinee’s conduct and behavior since their last examination or after they were placed under supervision. The examination can cover criminal, sexual, re-offending behavior or undetected crimes and violations. If specific information concerning improper behavior is sought, a brief statement covering all issues of concern should be provided to the examiner prior to the polygraph examination.

 

MAINTENANCE: Covers supervision or clinical issues including adherence to rules and regulations, honesty, violations of supervision or treatment conditions. If specific information is needed, a brief statement of all issues of concern should be provided to the examiner prior to the polygraph examination.

 

FULL DISCLOSURE: If the client has admitted to the instant offense, this examination covers all deviant sexual and/or criminal behavior in the examinee’s lifetime prior to, but excluding the instant offense for which they were charged and any subsequent re-offending behavior.

 

RE-TESTS: A polygraph examination re-test is administered in the event the examiner is unable to make a decision, or the cooperative examinee can not complete the polygraph examination process. A re-test examination will be administered at no cost to the examinee or paying authority. Re-tests the result of an examinee attempting to defeat the purpose of the polygraph examination will be required to pay for an additional test fee.

 

REFERRAL FORMS: Supervision officers, treatment providers, attorney and other requesters are supplied with several Referral Forms to request polygraph services. It is suggested that Referral Forms be completed and FAXED to 940-383-2240, or E-Mailed immediately after a prospective examinee is informed of a polygraph examination requirement. This early notification will permit the polygraph examiner to ensure that ample time is allotted for the required polygraph examination and permit time to discuss the nature of the examination and obtain any additional information or documents that might be required. Early scheduling ensures the examinee is able to select a date and time that fits their work schedule.

 

ISSUES OF CONCERN: If a requester, supervision officer, attorney or treatment provider has specific issues of concern to be covered during the polygraph examination that information should be provided to the polygraph examiner prior to the examination being scheduled.

 

EVALUATIONS: There are several evaluations that can be made concerning the results of a polygraph examination.

 

NDI: No Deception Indicated- is the evaluation applied when there are no recorded physiological tracings that indicated the examinee attempted deception to the relevant questions; and there were no suspicions or indications of countermeasures.

 

DI: Deception Indicated- is the evaluation applied when there are psychological recordings that indicated the examinee attempted deception to the relevant questions during the polygraph examination and the recorded physiological responses were indicative of attempted deception.

 

INC: Inconclusive- is the evaluation applied when no evaluation can be made as to either NDI or DI. This outcome might be the result of physiological or psychological problems.  Typical causes are the examinee was not physically fit or they were mentally distracted at the time of the examination, the examinee was not alert, the examinee moved during the examination or the examinee attempted countermeasures to defeat the purpose of the examination.

 

NO OPINION: This is used when an evaluation was not possible because of insufficient recorded data, the examination was not completed, and the examination was stopped by either the examinee or examiner prior to completion of the testing process.

 

SCHEDULING OF EXAMS: The examinee should call and make their own appointments. If the supervision officer, attorney or treatment provider has reason to believe the examinee will not call, or has trouble making appointments, the requester can schedule a polygraph examination by calling 817-909-3411. If the requester makes an appointment for the examinee, it is suggested that the requester call for the appointment with the assistance and with the examinee nearby to ensure the date and time are agreeable with their schedule, unless the requester will bring the examinee to the appointment.

 

PAYMENT: All appointments for polygraph examinations should be made as soon as possible to ensure a time period is available. Appointments are made to schedule a period of time. All polygraph examination charges are determined when the appointment is scheduled. The only methods of payment accepted are cash and money orders.

 

CANCELLATION POLICY: A polygraph examination appointment must be cancelled at least 48 hours prior to the agreed appointment time to be considered an excused appointment. If an appointment is not excused at least 48 hours ahead of time, payment must be made for the missed scheduled examination time.

 

LATE ARRIVAL TO AN APPOINTMENT: If an examinee arrives late to their scheduled appointment all efforts will be made to conduct their polygraph examination, if time permits between their appointment and the next examinee’s appointment. Otherwise, the late examinee will either be required to return after the last scheduled appointment for that day, or they will be rescheduled for another date and time.

 

TELEPHONE CONTACT NUMBERS: Every requester, supervision officer, attorney and treatment provider must provide their office telephone number, FAX numbers, pager numbers, cellular telephone number and any other possible contact numbers. The telephone numbers are required to permit contacting them in the event additional information is needed at the time of, or during a polygraph examination. These telephone numbers will not be provided or released to any other individuals without permission.  Having these numbers on file can prevent the rescheduling of a polygraph examination for the lack of information, clarification of facts or other incorrect information.
